(as the patient),

After recently having a hip replacement operation in August 2012 at Calderdale hospital. I would like to thank you for the excellent care I received from all the staff on ward 8B during my stay in hospital, and for all your efforts to make me well again, which I’m glad to say, have been successful.

I would also like to thank the physiotherapy team who have visited me in my home post op, and provided an excellent service.

I am very grateful for all the help and care I have received from the NHS.
