I used the early supported discharge team from the stroke unit at Mount Gould, Plymouth.
The physiotherapist visited my home every day for a period of time to assist with my walking. Prior to coming to my home and before I was discharged, the physio worked with me in the hospital so as to continue with a seamless service. The work by both the physio and her assistant (Sandra Shaw and Sharon) was of a very high standard and help greatly with my recovery.
They concentrated on physio and in particular walking but if needed they involved others such as an OT to help with equipment and kitchen activities.
I was very pleased with my care overall.
