"Not following up on agreements "

(as the patient),

I had attended for roughly 1 year and there had been two agreements reached at the end of two separate sessions.

Both times the referrals had not been made to the appropriate agency. There had been no recording of the agreements and this caused slight annoyance on the first occasion and distress on the second. It led to conflicting advice from two different consultants and left me feeling quite stressed and anxious.

If there was some kind of procedure in place to get a summary of the end of the consultation in writing for both parties records, then this would lead to a much smoother recovery pathway.

When people attend these consultations they do feel quite stressed and drained at the end of them, and it would be good practice to have a summary of what was agreed.

I do understand that medical records contain omissions and inaccuracies as staff can be juggling many things at the one time and memories of events through the day may be sketchy in certain aspects when it comes to recording them. This is a widely accepted phenomena in recording medical notes although not so well known by the general public.
