"Unhappy that my key worker changing"

I am sorry that the changes to Lincolnshire Partnership NHS Foundation Trust Drug and Alcohol Services have caused you to have a change in key worker. This is due to a total restructure of our Drug and Alcohol Services. We have recently been commissioned to provide a new treatment system that is fully integrated across both drugs and alcohol which has led to a total service remodelling exercise.

It is with regret that this has meant the unavoidable movement of some of our key workers in the geographical areas they cover. It is regrettable that the restructuring of services has affected continuity in your service delivery through this change. The new service model has a treatment philosophy to maintain the new key worker throughout the remainder of your time in treatment with our services which I hope will provide you with the stability you are seeking. A hand over between your old and new key worker will take place and the information contained within your treatment records will allow for a smooth handover of your treatment. Thank you for bringing this to our attention as feedback is important, particularly at the early stages of new treatment systems.
