What I liked

The friendly care and attention for my TIA stroke recovery and monitoring while in NMUH stroke recovery unit, Pymes Building, all female ward. Males kept on a separate adjacent ward. My stay on that ward lasted two weeks. However, my discharge experience is explained below.

What could be improved

Hospital discharge unit communication with Home Care Services needs more lead time for home care services to be put in place. Often times the patients are dropped off home with care packages ordered with a start-date but due to short notice, the care package not in place until the following day or next day. Patient is left stranded without necessary care assistance. More notice of discharge date from the hospital to Care Services would help to alleviate this problem. Also, some delays occur in medications being ready from hospital pharmacy in time to syncronize with home transport service. Home transport service do not like to be kept waiting as may have several patient home drop-offs to make in one trip and one patients' prescription delay affects the whole patient home delivery schedule.
