"Poor discharge of my father at Whiston Hospital"

About: Whiston Hospital / Older people's healthcare

(as a relative),

My father is 78 yrs old. Admitted Friday eventually diagnosed with Pneumonia and Pulmonaly Embolism on Tuesday, when treatment commenced.

He was discharged at 8pm on a friday- he lives alone. He has no care plan, has not been assesssed for social services care and was given a bag of drugs with no information apart from "it's on the label".

There was poor communication with his family and the delay in getting his take home medication was blamed on "the strike"

Thank you very much for submitting this story.

I'm very sorry to read about your father's experience, which was obviously unacceptable and must have been very distressing for both your father and the family.

The three Local Involvement Networks (LINk's) that cover Whiston Hospital, (Halton, Knowsley and St Helens LINk ), produce a regular report based on the comments received about services at the hospital. This report is presented to the monthly Patient Safety & Experience Committee at the Hospital. We will pass on your story in our report to the hospital and ask them for a response.

We really appreciate you taking the time to tell your story.

regards

Dave
