"Discharged from hospital without a discharge letter"

About: Musgrove Park Hospital / Urology

(as a relative),

Husband was discharged from hospital without a discharge letter after a prostate operation. This led to not knowing how to use the catheter correctly for a day (discharged on Sunday) All other aspects of care and treatment were fine.
