"My appointment was cancelled but nobody told me"

About: Devon Access and Referral Team (DART) Tiverton and District Hospital

(as the patient),

I called DART to arrange an appointment at Tiverton Hospital.

However when I turned up for my appointment at the time and date I was told I was not on the list. When the receptionist looked in to it she said the appointment had cancelled.
