Today a clerk in the maternity department has given my daughters notes to another patient who has gone home with them. The clerk couldn't have cared less but my daughter is being induced on Saturday and now has no notes. My daughter had a stillbirth at this hospital last year so this is very serious. Plus a stranger now has all her medical and private details.

I apologise that our response is so late but wanted to assure you that the Trust in fact takes breaches in patient confidentiality very seriously and I sincerely apologise that you and your daughter were given the impression that we could not be bothered.
