"the sharing of information at the eye clinic"

I have been attending the diabetes eye-screening clinic annually for six years and on each visit , my feet have been examined at the same visit to check on their condition with regard to my diabetes, after every examination I have been told that due to the results of the examination, I will no longer have my feet checked-out at the eye screening clinic but that I will in future be examined by a Podiatrist.

It would appear that I am the only person to be given this information as I am told the same thing at each attendance.( that is 6 years). So it would seem that the information is not being passed to the relevent departments.
