"Near miss overdose on pregnant woman"

About: Leeds General Infirmary

(as other),

My partner was nearley given a second Injection of anti coagulant drugs, which was equal to the first correct dose. If she hadn't said "Is that like the one earlier" it would have been administered. she is 39 weeks pregnant and was suspected of having a clot on her lungs.

Message from the Patient Relations Manager at Leeds Teaching Hospitals NHS Trust.

I am sorry to hear of your concerns. Unfortunately I have been unable to determine from your comments which particular clinical area your partner was cared for and therefore I have been unable to address these with the individual manager concerned.

Should you wish to discuss these issues personally please contact the Patient and Advice Liaison Service at the Leeds Teaching Hospitals NHS Trust on 0113 2067168.
