"Difficulties in coordinating after care "

My mother who sadly passed away in April was admitted to Royal Gwent Hospital in January where she received numerous tests and eventually a diagnosis which was terminal cancer. The consultant and assistant Surgeon were exceptional, kind, practical and caring as well as accommodating.

However, the coordinating of care following discharge was poor and at times non existent. Without my own links I would not have coped and her last few months would have been even more traumatic they were.
