My neice entered Lister for a routine operation. It went horribly wrong, she needed blood to survive, it took 40 minutes for the relevant person to get it to her. She died several months later having never regained conscientious. She was 30 years old, fit and healthy. It should have been a simple procedure thousands of which are carried out every year without a problem.

A thirty year old girl had her life taken from her. It wasn't because of spending cuts or lack of staff, it was because somebody did not do something quickly enough to save her life. How tragic this is. This person probably isn’t even aware of the catastrophic results.

We are truly sorry for your loss - please accept our deepest condolences.

Whilst we know that nothing can bring your niece back, this incident will be investigated thoroughly. In part because your niece deserves nothing less, but perhaps just as importantly, we need to ensure that any lessons learnt are shared with our staff.

Every week hundreds of operations are carried out at the Lister without a problem, so what happened to your niece was an incredibly rare event. That should never be seen as a reason, however, why the tragic circumstances surrounding your niece's operation aren't investigated in full.
