What could be improved

my father passed away on monday 9th of August,and it is now thursday 12th August, my mother and i are still waiting for the relevant paperwork to register the death of my father.Despite several phone calls the doctor who didnt correctly fill in the form required cannot be traced. Apart from the stress this is causing us, we cant seem to find anybody at the hospital who will tell us the truth about what is the delay. If this is normal procedure and 3 full working days is the normal delay then thats fine . Personally i feel we are being kept in the dark about something.
