What I liked
My Dad was admitted via accident and emergency with what they said was a urine infection. As he had only just been to the toilet he didnt have the need to wee, so he was catheterised. He was fine and doing crosswords and walking about everywhere.He was in a week and didnt see the urologist at all, but was discharged with the catheter in place a week later with an appointment for outpatients. 2 hours later he had to be blue lighted back in due to breathlessness. Put on coronary care where the treatment was brilliant. My dad died there a week later. He had been on water tablets for years and on admittance they stopped them. This caused fluid to build up in his lungs and his heart couldnt cope. When he was discharged they put him back on them....too late. He should never have been discharged in the first place.
What could be improved
More information given to family members and better communication required between shift changes.Also for them to read a patients folder to better understand their needs as a person and not just as a number
