"Extremely upset and angered by my treatment and..."

After waiting over an hour to be seen, I was turned away because my GP hadn't put my date of birth on my form. I have been to the clinic previously for blood tests and did not need ID therefor I did not bring my passport. I had my bank card but it wasn't enough as it didn't have my birthday. I was afterwards told by my lovely GP that this shouldn't have been an issue as my NHS number was on the form and the nurse should have looked it up.

What really agitated me was the nurses lack of remorse or consideration. They suggested it was my fault for not having checked the form and acted as if it was ridiculous that I don't check these things. I have no idea that the birthday was so important to them and I trust my GP to fill out a form. They did not apologize once.

I'm in the process of trying to find a route to formerly complain but I can't find that on this page. When I do I will be looking for some compensation (I want to have my blood taken without waiting an hour and 20 minutes) and I want to know why they do not check your forms for these sort of issues upon arrival.

What a horrible nurse, I will be returning to find out their name as I didn't ask when I was there. Truly upset and ruined my day.
