Several members of staff failed to identify that my baby had passed away during pregnancy whilst I was an inpatient over night. A midwife noted they could not hear my baby's heartbeat or 'the source' aka the placenta! Multiple members of staff documented that I urgently needed a scan but i was discharged without one. My blood results were completely missed, twice, which clearly highlighted one of my levels was significantly raised and was a clear indicator my baby had died but despite all this I was discharged the next day. Staff tried to make decisions for me as to what my options were in relation to 'expelling' my baby - despite what I had clearly advised I wanted, they tried to back me into a corner several times taking no regard for my preferences. Complete lack of communication all round, in different departments - dragging out such a horrific ordeal for me. 2 months down the line I'm still dealing with their incompetencies.
