(as a relative),

Nearly two years ago I wrote about the health board's investigation into my son's death. Two years on they are instructed by the SPSO to apologise. Three weeks ago they send an apology letter to a completely random address. You couldn't make up this sort of incompetence up even if you wanted to.

Dear Tigermum,

I am so very sorry that the apology letter was not properly addressed. The Scottish Public Services Ombudsman contacted us yesterday to point out this error, and we are rectifying this today.

This was entirely our fault and we will ensure that this is not repeated in future.
