"Funding for support Worker"

(as the patient),

I was seen in November 2015 for my Active Review; at that meeting the staff member was given a copy of my Support Plan in which I requested funding for a Support Worker. The request for a Support Worker was initially denied; however I appealed and was then awarded £30. 00 per week to cover my Support as requested. Apparently that award was for a period of 3 months only but I was only informed of this in a letter dated in August 2016.

I have still not received any of the Direct Payments allocated for my Support Worker despite my request in November last year.

In March 2015 I rated my care after as 'Excellent' on the feedback leaflet.

Today I would give DPT a 'an Extremely Poor' rating" to the point of being 'negligent in their duty of care' towards me.
