"Discharge again"

I am warning all of you that you should not trust the discharge team. My husband was discharged today and taken to care home there was suppose to everything put into place but nothing at all was done I was told that he would have 1- to 1 care 24 hours a day for the first week at least this was not done by the discharge team I was told other things was going to happen but nothing was done by them. They are a department who walk around the hospital pretending to be busy. I am writing to make sure that you should not trust them. I am making an official complaint tomorrow to the hospital. Be warned
