Cleanliness. Specific examples soiled bed linen, dirty floors. Building maintenance had been badly neglected Specific examples windows and partitions held in place by gaffers tape. Communication from medical staff. Specific example details given to doctor completly garbled when read back

Staff overworked and badly organised.
