What could be improved

I visited my mother to find that she had been discharged and sent back to her care home. My mother had asked whether I had been informed of the discharge and she was told that I had, which was untrue. There were no discharge papers and her medication had not been arranged. Futhermore, the carehome were not allowed to assess whether her medical condition could be properly managed at the carehome.

Overall, a disgraceful way to treat an old person.
