"Management of Significant Adverse Event Reviews"

About: Crosshouse Hospital / Trauma & orthopaedics

(as a relative),

My motheer was admitted to Crosshouse Hospital via A&E in May 2016, she fractured her hip and her wrist, she was put down for emergency surgery that night or following morning, she was then admitted to a ward.

The surgery did not go ahead that night, it was planned in for the following 2 day days and again cancelled on every occasion. Mum was given morphine to manage her pain, she was finally taken to Theatre a few days later and the family were called to say that mum had died unexpectedly in Theatre.

as you can imagine we were in shock, as NHS Ayrshire & Arran have a procedure called Management of Significant Adverse Event and Reviews and as mums death was in Theatre, under Anaesthetic and unexpected then a Datix and SAER should have been completed.

On contacting NHS Ayrshire & Arran to ask when we as a family would be consulted on this, it would appear that this procedure has not been carried out, I can't ascertain this for certain but have been informed that it doesnt look like one has been done.

Please accept my sincerest condolences on the loss of your Mother. It must have been a very traumatic way to hear about your mother’s passing. It is totally understandable that you were both taken by surprise and shocked.
