My father was an in-patient for 5 daysfrom10/2/10 until today. During this time he was administered intravenous steriod treatment over 3 consecutive days. He was on a neuro-medical ward where a number of patients have cognitive impairement. He did not have any form of patient identification (wrist band) during his stay and there is no patient identification at the bed area.This constitutes a serious risk of potential drug error and must surely contravene Trust policy on safety and drug administration. I intend to write to the Chief Executive regarding this dangerous practice.
