"Appointment Times Not Kept"

Appointment 4pm. Arrive 15mins early in order to be on time.
There is a board that tells you they are running 45minutes late. Total wait time 1 hour.
Why not text out to patients of the day and advise of long delays. Or should we assume that there is always a delay of 30-45minutes when an appointment is scheduled and arrive 45mins late in order to be on time?
