"Lost notes in A & E"

About a year ago this gentleman’s mother (aged 84) was taken to A & E. Paramedics were excellent, and all treatments were done. The notes were lost in A & E, so tests had to be redone, then after a shift change the notes were lost again. So the tests were done again.

They have no complaints at all about the treatments (only praise), but can’t understand why records were all on paper and so easily lost. Retesting is not only a nuisance, but also costly for the NHS.

Thank you for the feedback regarding this gentleman’s visit to A&E we understand from Healthwatch this was over a year ago.

Our A&E department have made significant changes since their last visit and we continue our improvement journey.
