Lack of communication between the staff and with the relatives ( except for one nurse ) . Medication being administered and not being recorded on the record of care file , i was asked by the nurse to inform the next nurse to oversee my father to record it on the record as this was not at the bedside ( the medication being a Salbutamol 2.5mg nebule ). When I asked to see my fathers record of care sheet , which had been absent from his bed for the time of his admission to the ward , i was given the runaround after i stated that i am a health care professional and still not privy to the stated above . After securing a bed at the Royal London renal unit , a wait of 4 hours before myself and family left at 8pm after being informed that he would be transferred later on . The next morning when i did contact the Royal London to confirm the transfer i was informed that this was not the case . On visiting Newham later on during visiting hours , I was informed by one of the nurses that reason not being transferred the previous evening was , quote "he took a turn for the worse " . When i asked my father to confirm the above the denied that this had taken place , upon which i requested his record of care documents , which once again were missing from the bed , whilst i could see many other patients bedside having being present on site. Once again i was given the runaround by a number of nurses and prevented from accessing the audit trail . It took an another week to transfer him to the indented destination at the Royal London ( Whitechapel ) , on my first visit there I had access to medication regime , nursing staff who were very well informed on his condition . My father has visited Newham General on two occasions and both have been totally unsatisfactory.
