"Terrible communication"

About: Wexham Park Hospital

My mother has spent the last 5 days in two wards in Wexham. She was admitted as an emergency but is no longer in pain. She does however have dementia and terminal cancer. At a meeting today we we told the plan of action, this then changed but no one told us until someone turned up to transfer her to Upton. Thankfully my sister was still there. On the ward the nurses ask my mum questions about how she feels, her toileting etc and use the answers to complete the notes. She has dementia! They should be observing her not relying on her.

I see that you wrote your review on 15th Jan but it hasn't been available to me to respond until 23rd Jan.

I am very sorry to read that we haven't communicated with you regarding your mother's care and that the agreed plan changed without anyone informing you or your sister. I'd really like us to help resolve this and investigate what happened.
