"medication review"

I was contacted in December to make an appointment with the practice nurse to have a medication review. When I made an appointment I was called back to say that the review had taken place in my absence via computer. It was subsequently decided I did not need an appointment.

If this is to happen, then part of the review should involve the patient being informed that the review has been undertaken and what the findings are.
