My sister is aged 60 and a vulnerable adult with Mental Health issues. She was discharged home on 23/12/15 -2 days prior to Christmas without a robust care plan .I am her sister,a doctor,who lives 100 miles away .I last saw her on Ward 16 on 16/12/15 when in my opinion she was in no way near being eligible for discharge as she was immobile .
I received a phone call from a staff nurse on the afternoon of 23/12/15 saying she would be discharged that day .I assumed measures had been put in place to provide fresh food etc. -it then materialised that this was very far from the case !
My sister then suffered a torrid few days with a new care agency and her mental health worker doing their best to care for someone who was unfit to be discharged home in a vulnerable state.....She was readmitted to Blackburn Hospital on 28/12/15 where she remains.
I am totally disgusted with the inability of staff on Ward 16 to make a safe 'Risk Assessment' of a patient.
