"Breach of confidentiality"

(as the patient),

I recieved a letter informing me of a breach of my confidentiality in the week before Christmas. A notebook containing information about my identity and clinical information was left at a property by the home treatment team. I was last under their care in September 3 months before this letter was recieved. I contacted pals the day I recieved the letter and only got an answer phone. Left message, sent an email and in subsequent days tried phoning again to no avail. Despite claims someone would be in contact 3 working days after making contact I have yet to hear anything. I feel this service is a complete joke.

I'm extremely concerned as to what happened with this incident and I want answers. I now have to try and enjoy Christmas with this in the back of my mind. It simply isn't good enough. A really poor service which I feel totally let down by. If they can't get confidentiality right what can they?

Dear Angrypatient95,

I am very shocked and concerned to read of your experience and I am very sorry that you have received such a distressing letter prior to the Christmas period. I am also very sorry that you have not received prompt contact from myself during this period.

I would very much like to get in contact with you to ensure that I am able to discuss how I would be able to look into this matter and also to apologise for the lack of contact and to hopefully follow this up also.

I am available for today (30th December 2015 and for tomorrow 31st December 2015). The I will return on Monday, 4th for normal office hours Monday-Friday from 9am until 5pm.

I do hope that we can speak in the near future and again I am sorry for the experiences you have taken the time to share with me.
