"Help manage two long term health conditions"

After a resent diagnosis of a neurological condition on top of arthritis I felt as though the two conditions were fighting each other. I am treated extremely well by my local GP surgery, including GP's who have seen me only in emergency appointments. So I knew anything on top of that would be a bonus! My GP referred me to the local REACH team where the OT talks you through the process and health professionals visit you at home to help make things easier. First visit was from a speech therapist which was due to swallowing issues and speech problems. Then I met the Physio who explained how the conditions were effecting me and how to strengthen my joints. To the degree I had a holiday in Germany and managed to walk more than expected! I was then seen by the OT who managed to not only make my house safe but also helped me to better manage my physical health by talking through my preconceived ideas on resting, which seems very basic but helped me immensely!
