Whilst my father was a patient in Tameside Hospital, communication was very unreliable to have correct people at varying meetings especially as the patient’s doctor’s surgery comes under Oldham Pennine Trust and Tameside Hospital team don’t work the same or deliver same service. This is very frustrating on discharge and because of differing boundary arrangements hard to have everything in place.

The hospital social worker was good. The availability of paper work could be improved.

Thank you for taking the time to post these comments through Healthwatch.

It can be difficult to arrange meetings with other parties in a timely manner when patients are in hospital.

We are working hard to address a more integrated approach with all aspects of health and social care and are trying to adopt a new way of working which should improve the frustrations you experienced. This is a long term strategy that the Trust is focusing on at the current time.
