Notes lost or inaccurate - sometimes written on hands of staff.
No communication between staff and very little between staff and patients.
Clear lack of midwife training in basics like cannulation and more technical equipment which is either ignored completely or fixated upon to the exclusion of anything else.
No interest shown in actual birth or patient's experiences during it.
Improper use of opiate based pain relief during birth.
Deceitful and disinterested PALS team resulting in an offensively incomplete complaint response.
This department has survivors and they should know how lucky they are.
