What I liked

I came in to the emergency care on a Sunday and there was no Doctor on staff. The Nurse was very capable, and in fact, diagnosed the problem perfectly. She did a great job. She provided all the information I needed to get additional medical assistance and was gracious and attentive. Well done.

I neglected to get a copy of my medical write up for my records. I am an overseas patient. I live in Boston, MA and need this information for my insurance company. How do I go about getting this information?
