"Discahrged 88 Year old with Dementia and a..."
About: Queen Alexandra Hospital Queen Alexandra Hospital Portsmouth PO6 3LY
My father was discharged from D 1 after 6 weeks with assurances from the hospital social worker that district nurses would visit daily to clean his neck and check his Miami neck brace.
They would visit until Care workers could be trained to offer this service.
This is completely wrong.
District nurses do not offer this service and the District nurse I finally spoke to says this error has occurred before.
I have spent two days trying to get the District nurse to visit my father with a sore, dirty neck.
Today the D1 ward sister was incredible, getting in touch with the District nurses for me and getting them to make a visit.
The only visit they are prepared to do.
Luckily they coincided with the Carers and was able to "train" them.
This misinformation is appalling.
Why were we given wrong information that caused much distress to my father and family?
Do your social workers not know basic services available on discharge?
My father was also given a double dose of medication for two weeks when his medication charts were written up incorrectly.
How can that happen!?
No lasting damage to heart or lungs apparently...
Staff on D 1 were kind and caring but there does seem to be a lack of communication between teams and no one prepared to show initiative.
Thank you to the busy ward sister who went out of their way to help sort matters today.
They are exceptional.
