"No follow up after traumatic delivery"

I was induced on 18th August 2014 and gave birth naturally on the same day. After the delivery I lost 4 units of blood and was rushed to surgery in order to save my life. One year on I have not had a review of what went wrong on that day. I requested this from my health visitor but did not hear anything back. Even though I'm thankful to the hospital for reacting so fast in order to save my life, I would really appreciate a follow up with the team involved in the care about what occurred on that day and any medical implications of it to me.
