About: Inclusion Matters Wirral

(as the patient),

I was expecting an appointment for August. On my return from holiday I received a text reminder to advise me I had an appointment in July - as I am abroad I am unable to phone but asked my sister to call field road to leave a message.

I would be really grateful if you could let staff know why I wasn't at that appointment and hopefully I can arrange to see someone on my return as discussed.
