Hospital made mistakes which they refuse to admit. I firmly believe those mistakes directly caused my father's premature death. Never use informal complaints because when it is eventually done they dictate the whole process and change it between arranging meeting and meeting taking place. They only want pats on back for staff and will not be honest about what went wrong. Neither will they minute anything said which show their failures. Formal complaints result in questions not being answered, emails ignored, phone call promises ignored. Also they rely heavily on people at fault providing answers for you as opposed to actually investigating properly. That hospital does not know the meaning of patient care, openess or honesty. For compulsory question re Department visited. It is answered as "don't know" because there are several involved, not all are listed and you can only pick one.
