"Patient centred care seems to be lacking"

My father is currenlty undergoing treatment within Weston Park. Although I have no doubt that the treatment and care he is receiving on a personal level is of a good standard, the overall quality of patient centred care (i.e. co-ordination, involvement and information sharing) with my fathers supportive network is sadly lacking. What's even more concerning is that the nursing and support staff looking after my father on a day to day basis are also unable to give clarity around the co-ordination of his treatment plan - its as if there is no multi-disciplinary handover or cross-over taking place. Up to now, its been difficult to ascertain the scheduling of consultant visits in order to seek clarity around the co-ordination of his care with responses narrowing this down to either a full morning or full afternoon. Not ideal when your trying to work, support other family members and fit in visits as and when possible. Had my father been co-herent enought to ask and understand his consultations this would not be a problem, sadly this has not been the case due to the medication he is receiving. I fully appreciate how stretched resources are but its increasingly frustrating whilst trying to find out the wider implications of my fathers illness when there is a lack of co-ordination and knowledge between each individual member of staff despite them all carrying out their own roles with such compassion.
