"Inaccurate discharge sheet"

This is to follow up a story already posted by me on this website.
I flagged that I received a Discharge Summary that was highly inaccurate. The Hospital took ten days to produce an amended version, which is equally inaccurate. Most of the inaccuracies highlighted were not corrected.
The summary mentioned I was discharged to my usual place of residence, however I remained at the hospital to be transferred in an ambulance to Chelsea & Westminster Hospital who were better equipped to look into my condition. If I was discharged then I was discharged straight into the care of CHW, not a usual place of residence.
The summary also said I was discharged for GP follow up. Again wrong. Outcome was that I was detained overnight to be transferred to CWH as transfer could not be completed on that day (that is what I have been told).
It said the incident occurred at my home. In fact, the incident occurred at my doctors’ surgery and it was one of the doctors who called the ambulance.
It also said I was discharged, but reluctant to stay at home. In fact, I stayed at the hospital overnight on the direction of the team at CWH as a transfer could not be arranged on that day. I have never been told that I was discharged on that day – on the contrary, a member of staff told me (after consulting with her superior) that I could not collect some personal items as in that case the Hospital would have to discharge me and I would not be able to use the ambulance the next morning to be transported to London.
Needless to say, I am extremely disappointed with the level of care and service I am receiving from the East Surrey Hospital.
I suggest, once again, that this Discharge Summary is corrected to fully to reflect the details I have given above because in the current form it is plainly inaccurate.
