"Disappointed and concerned..."

My husband, who has terminal cancer & limited life, was given an emergency appointment in March to see what was the cause of loss of vision in one eye. It was a cataract. It was at the end of the day (4.15 isn) and a machine had been switched off but it was switched back on when the operator realised his condition and the measurements were taken and we saw the consultant/assistant. Disappointed: He was given a date of 20 June 2015 10am for the operation and at 9am on the morning he was due to go in we had a phone-call to say that the operation had been cancelled as his opthalmology notes could not be found. I was told that we would be called back on Tuesday 23 June to let me know what was happening. On 26 June I called the opthalmology department to find out that a date in late July had been sent - not for the operation but to start the procedure again. Concerned: His opthalmology notes have been lost - which is appalling! These are highly personal notes. I expressed my surprise that the department, considering that it is not in the main hospital, does not keep a back up copy or that the records are not on computer. Clearly this is not acceptable. Under the circumstances I feel that he should be offered a more speedy service if the notes cannot be found. And a written apology that the notes have been lost! He has been seen for diabetic retinopathy and had laser treatment and apart from some long waiting times there have been no other complaints.
