A relative was recently admitted for intravenous antibiotics and IV fluids - hand hygiene by the nursing staff was below an acceptable standard, and infection control measures disposing of bodily fluids was undertaken without gloves or hand washing. Communication between departments, wards and multi disciplinary teams was below an acceptable level resulting in medication and fluids not administered as prescribed, and a delayed discharge.
