"Lack of consistent treatment"

Case notes should be read and families consulted for signs of deterioration before reducing or removing key medications from a patient's treatment and returning them to the community with little or no support. Prevention is cheaper than cure both in terms of patient health and resource use when a relapse occurs. The approach of the unit appears to be 'what can we get away with', not what is best for the health of the patient and their families.
