"Complete lack of support for carers at discharge"

My dad is currently in the William Harvey as a severely disabled stroke patient with suspected cancer. The hospital have told my mum (his full-time, unsupported carer) that he has to leave hospital but have not lifted a finger to help with arranging the support that he now needs. There has been a complete lack of communication, misinformation and chaos - no-one has assessed his needs, discussed home support with my mum or kept her informed of what is going on, yet at the same time they are pressuring her to take him home. There is no link with community services, there has been no referral for support and the care is disintegrated. My mum, his carer, is going spare with the stress and anxiety - carers do the most amazing and essential job for our loved ones and save the NHS millions in costs - they don't deserve this abandonment!
