"discharge procedure for weekends of terminally ill..."

About: City General Hospital

Our father suffering from Alzheimer's and cancer was admitted to the hospital on Tuesday and the subsequent care he received was excellent In both AMU and 230 ward. However it was felt further treatment wold not be helpful and he was discharged on Saturday evening. No follow up advice was given regarding support or contact for the remainder of the weekend. No information was provided in a form which could be given to agencies such as out of hours GP service in event of any concerns over the weekend. Details of his discharge are to be emailed to his GP but this will not be available to the out of hours service during the weekend. This has left us feeling disappointed that the weekend discharge procedure did not meet the same level of excellence as the care he received prior to discharge.
