Dear Sir / Madam, I am sorry that you have been so frustrated by our failure to diagnose your asthma correctly. I am also sorry that you weren't listened to as you would like and that you haven't been included nor appropriately involved in decisions about your care. Such a shame when we know that someone living with a long term condition will have a wealth of expertise and strategies on how to make life a little easier. I I will pass on to the team - however if I had your name I would be able to personalise your story and this is immensely powerful in helping our teams to reflect and learn.
