"I did the self referral form for antenatal care. ..."

I did the self referral form for antenatal care. Someone called to confirm my GP and other information. A week later I was still waiting for my appointment letter, so I called. It turns out that the letter was sent to an address I haven't lived at for 4 years! What was the point of me taking 15 minutes to fill out a long questionnaire with information like my address, phone number, GP ect, if they were just going to copy and paste in old information?? I am so glad I called to follow up, or I would have missed my appointment. I am not happy, and feel my baby's care is already off to a poor start. I am seriously questioning my choice to have my care with Northwick Park and not Queen Charlotte's and Chelsea Hospital.
