My gran got admitted just after christmas to the rvi which then transferred her to freeman just after nye. She got told she had a serious illness. And that the freeman would deal with it more. She was on oxygen at all times. I contacted the freeman this morning to see how she was doing, i spoke to her own doctor who alerted me that she was returning back home today. Ive came in from work today and she doesnt look like she should be sent home this early

We are very sorry to hear your concerns about your Grandmother’s recent discharge from The Freeman Hospital. Unfortunately, without further details it is difficult for us to review what happened, and we would be very happy look into this further for you. Please could you contact Paul Anderson, the Trust’s Patient Relations Manager on 0191 223 1382 and we can look at this for you? Our expectation is that all patients are assessed, to identify their on-going care needs on discharge, and that they are provided with the necessary care to support these needs in the community. Patients can be discharged once the acute phase of the illness is in hand and it is felt that recovery can be supported by our community care teams. Agreement should be reached with the patient prior to discharge to establish the appropriate support. If we have not achieved this in your Grandmother’s case we would want to review this and do apologise if this has caused distress to yourself or your Grandmother.
