"My wife's cancer was diagnosed too late"

My wife Sarah was seen by the radiologist, diagnosis benign polyp, no internal investigation carried out.

Three months later Sarah had a scan and ultrasound carried out by the registrar, diagnosis benign polyp, again no internal investigation.

A week later Sarah had operation to remove this polyp (a day procedure). This procedure failed - they could not remove the polyp, Sarah was told the polyp had been lassoed but was tough and rubbery and would have to come back in three months when a decision would be made on how to proceed.

Again no scrape or biopsy was taken, Sarah was told by the surgeon the polyp was benign, a letter to Sarah's GP from the hospital confirmed the surgeon's diagnosis.

Sarah was readmitted to the hospital after six weeks, when advanced cancer was diagnosed. She had a hysterectomy, recovered from the operation, then became infected with C. difficile and died.

I am proceeding to take legal action for lack of care and incompetence.
